Thank you for choosing Dentist On The Park. We are proud to be a small, privately owned dental practice with a small staff and cutting-edge technology.

Our small size allows us to practice better, more enjoyable dentistry and to create more personalized interactions with our patients.

The policies below are meant to help us provide the best care to the most patients possible. We prefer to not double-book our patient and to schedule enough time to provide treatment in an efficient and stress-free environment.

Factors that affect our daily flow and threaten the livelihood of our small business include:

  1. Patients who have not given us updated insurance before their visit

  2. Patients who run late for appointments

  3. Patients who do not confirm their appointments

  4. Patients who cancel their appointment without the courtesy of 3-business days notice

  5. Patients who attempt to receive treatment without paying their estimated copays

Scheduling

  • We reserve the right to charge a scheduling deposit for any appointment, including hygiene or treatment, or for patients who have missed or rescheduled appointments in the past.

  • We ask that patients running-late for their appointment contact us immediately. As a courtesy to our other patients, we reserve the right to reschedule an appointment if the patient is running more than 10-minutes late or if we feel that their treatment cannot be completed without disrupting the appointments of other patients.

When a patient late-cancels or doesn’t show up for an appointment other patients miss out on the opportunity to receive care. That lost time results in increased overhead costs for our small business and wasted professional hours.

Cancellations and Rescheduling

  • As a courtesy, we ask that you provide us with more than 3-business days notice when cancelling or rescheduling appointments. This does not include Saturdays/Sundays as we are not in our office over the weekend.

  • A late-cancellation fee of $50 will apply for Hygiene Appointments

  • A cancellation fee of $50-per-hour will apply for Treatment Appointments (3-hour appointment = $150 cancellation fee).

  • Cancellation fees will be added to the patient’s account and are expected to be paid-in-full before future appointments will be scheduled.

Payments and Insurance

  • Payment is expected on the day of service unless other arrangements have been made in advance.

  • We reserve the right to pre-collect a deposit for treatment in advance before scheduling the treatment appointment. The deposit will be applied to the treatment once completed.

  • A patient’s insurance is their obligation. As a courtesy, we submit claims to insurance companies. To do so accurately, we require that a patient’s insurance is verified BEFORE their appointment. We ask that patients submit their updated insurance 1-week prior to their appointment to allow time for verification.

  • If a patient arrives to their appointment without updating us of their insurance information, they may be asked to pay our Standard Fee, and will be reimbursed by their insurance in the future.

Patient Dismissal

  • We reserve the right to dismiss and discontinue treatment with any patient at any time. In the event of patient dismissal, the patient will be given 30-days notice and will have access to our team for emergency care only while they find a new dental provider.

  • Patients who have rescheduled 3 or more times with less than 3-business days notice will be subject to dismissal

  • Patients who No-Show for a scheduled appointment without notifying our office with be considered for immediate dismissal

  • Patients who cancel an appointment Same-Day will be considered for immediate dismissal
