New Patient Forms
Once you have scheduled an appointment, it’s time to complete your New Patient Documents.
We kindly ask that New Patients complete their intake forms before arriving for their first appointment. This gives us time to be prepared for you!
Our Location
We are conveniently located in the historic Willoughby Tower at 8 South Michigan Avenue, overlooking Millennium Park in downtown Chicago.
Driving & Parking
For convenient and discounted PARKING, we recommend downloading the SpotHero app and parking at Millennium Park Garage or Grant Park Garage. Book in advance to save $$$.

We are currently In-Network with the following insurance providers. We accept PPO insurance only. We do not accept HMO, DHMO, Medicaid and most Medicare policies. If you are unsure if your plan is accepted by our office, contact your insurance provider for more information.
No insurance, no problem! We offer an affordable annual membership which covers all of your preventative services.
